The One Step Ahead Fall Prevention Program provides free, personalized in-home fall prevention support, including a fall risk and home safety assessment, individualized recommendations, limited safety equipment when appropriate, and connections to community resources.

To be eligible, individuals must meet all of the following criteria:
- Be 50 years of age or older
- Reside in King County outside the City of Seattle
- Live independently and not in a nursing home, assisted living facility, adult family home, or hospice
- Be able to walk with or without an assistive device
- Have experienced a fall within the past six months that resulted in a 911 call

You can submit a referral using the One Step Ahead Fall Prevention Referral Form. Referrals may be submitted by healthcare providers, community organizations, caregivers, family members, or individuals themselves.
A Program Health Educator reviews the referral and contacts the referred individual within one to three business days to discuss next steps and, if eligible, schedule an in-home visit.
No, all services provided through the One Step Ahead Fall Prevention Referral Program are free.
No, there is no need to clean or make special preparations. A Program Health Educator needs to see how you move through your home in your everyday environment. Simply make sure walkways are safe and accessible during the visit.
Yes, family members, caregivers, or others who support the participant are welcome and can be helpful during the visit.
Yes, when appropriate. Based on a Program Health Educator’s assessment, a limited selection of safety equipment may be provided and installed. Equipment is available in limited selections and quantities, and installation is subject to program guidelines and may not be possible in all homes.
No, the program does not provide medical care or treatment. It does provide fall prevention education, assessment, and limited safety equipment when appropriate. It does not replace medical care, diagnosis, or treatment provided by your healthcare provider.
