Summary

The goal of this RFP is to seek an eligible organization willing to collaborate with Public Health – Seattle & King County (PHSKC) to assist Ukrainian refugees and humanitarian immigrants through health education and navigation provided by Community Health Workers (CHW). CHWs are an integral part of the community and serve as primary points of connection to culturally and linguistically appropriate health services. These services will include but are not limited to

  1. Bridging gaps in health information and literacy
  2. Connecting to health and social services
  3. Providing cultural connection via 1 on 1 care plan development and navigation. Qualified applicants must demonstrate success in proposed services and the capacity to provide services that are culturally and linguistically appropriate.
