Superior Court Forms

Below are the forms most commonly used by the King County Superior Court. However, this is not a complete list of all forms used by the court or by individual judges. If you have questions regarding which form to use, please contact the court.

Washington State Superior Court Pattern Forms

In addition, Superior Court uses many of the Washington State Pattern Forms, particularly in the family law arena. These forms and instructions are managed by the Administrative Office of the Courts.

Approved Pattern Interrogatories Forms

King County Superior Court has approved pattern interrogatories for use in automobile tort cases.  The standardized questions allow plaintiffs' and defendants' counsel to propound written discovery requests more quickly and easily.  These forms are located on the King County Bar Association website.

For family law cases, you can contact Family Law Facilitators for assistance. 

You can visit the Law Library of King County and request packets and forms through email, US-Mail or in-person. Their office hours and locations can be found here

 

To file a new case, you will need to have the minimal required documents for that case type and the filing fee or a fee waiver. To find out what documents you need to file your case, click here.

Filing a New Case

If you are filing a new case in person, you will need a Case Information Cover Sheet (CICS).

If you are filing electronically, you do not need a Case Information Cover Sheet. The E-Filing Application will generate a Case Information Cover Sheet for you.
