Print out the appropriate Care Plan Packet
If a child in your program has a health condition or behavioral health need, print out the appropriate Care Plan Packet below. If a family has provided a care plan signed by a licensed healthcare provider, skip to Step 3. Note: If you use one of the packets below, you do not need a separate Medication Authorization Form.

- Food Intolerance Care Plan Packet (112 Kb)
- Seizure Care Plan Packet (754 Kb)
- Individual Care Plan Packet (210 Kb) - Use this for health conditions not listed above or behavioral health needs.

Complete the care plan packet
Have a licensed healthcare provider and parent or guardian complete the care plan packet and return it to your program.

Print out the appropriate checklist
All items in the checklist are required for a care plan to be compliant with Washington Administrative Code (WAC).

Review the checklist for completion
Use the checklist to review the care plan and make sure all items are complete. If all items are complete, stop here.

What to do if a checklist is incomplete
If you cannot complete the checklist, the care plan is missing information. You will need to do one of the following:
- Return the care plan to the healthcare provider or family to complete the missing information.
- Print the pages you need from the Care Plan Packet listed in Step 1. Give them to the healthcare provider or family to be completed.
