We want your feedback on your permit experience. Please take our brief (less than 5 minutes) Environmental Health Services Survey. The survey is open until July 12 and available in multiple languages – use the language selection box in the upper right corner to choose your preferred language. Your feedback is anonymous and will be used to improve our services.

Before you register, please review these tips

  • Use a generic email account

    If this registration is for a business, consider using a generic email rather than an individual's email, in case of staff turnover (i.e. manager@businessname.com)

  • Save the confirmation email

    For future reference, save and/print the confirmation of registration email you receive from the system.

  • Write down your username

    Your username will be your email address; it will be all lower case.
