Children with Special Health Care Needs (CSHCN)
CSHCN services
A Public Health Nurse can provide you with Care Coordination services through home or community visits or on the telephone. Services are offered to families in King County regardless of income to:
- Weigh and measure your baby
- Listen to your concerns
- Help you with your child's changing needs
- Provide developmental screening and assessment of your child
- Help with concerns such as feeding, nutrition, growth, development and behavior
- Refer to local community resources and organizations
- Help locate financial assistance to pay for your child's health care
- Help you organize services
- Work with your child's health care and other service providers
- Help develop and maintain a network of services and providers
Services eligibility
A child (birth to 18 years) that has complex health needs or has a newly diagnosed complex medical condition and the family needs help:
- Finding or coordinating medical providers and a medical home
- Accessing local resources and/or services for the child
- Finding financial assistance for the child's health related needs
- Understanding and/or coping with the child's health condition
